Important DMHS Summer Camp, Team Tryout and Tournament Info

INFO FOR SUMMER CAMPS, DMHS VOLLEYBALL WEBSITE AND SAND TOURNAMENTS

• Register your e-mail and parent emails at www.dmhsvball.com visit www.dmhsvball.com to find up-to the minute information and updates about DMHS Volleyball events and news.
• Go to www.dmhsvball.com and click on the link to register for the Wolf Den Sand Volleyball Tourney 4/25 is the last day you can register and still receive a tournament tank top!
• Volleyball Tryouts at DMHS for the Freshman, Junior Varsity, and Varsity Teams are on August 11th.
• Go to www.dmhsvball.com and click on Summer Volleyball Camps and register for the June Camp 6/9-13.
• Stay tuned for more information about camps and sand tournaments at the end of July/beginning of August.

INFO FOR COMPLETING THE DMHS ATHLETIC PACKET REQUIRED FOR TRYOUTS FOR 2014-15 DMHS ATHLETES

• Go to the Desert Mountain High School Web Site, click the tab on the left that says “DMHS Athletics” then scroll down the page and click the link on the right labeled “Athletic Packet w/Physical Form” ALL of the info is required to be entered online and then printed FOLLOW THE CHECKLIST IN THE PACKET AND READ ALL OF THE INFORMATION.
• CHIROPRACTORS ARE NOT PERMITTED TO GIVE PHYSICALS FOR AIA SPORTS.
• There is a NEW Physical Form this year, be sure to bring the Physical Form from the 2014/15 Athletic Packet to the Medical Provider and have the Medical Provider sign two of the forms. You will be required to turn these in with the rest of your Athletic Packet in order to be cleared for tryouts.
• Be sure to have your completed Athletic Packet and Physical turned in no later than 7/17 to the DMHS Athletic Office. Before turning your packet in please contact Mrs. Wahlberg  at 480-484-7006 and/or gwahlberg@susd.org.
